Club Waiver

CONTRA COSTA BASS CLUB ("CLUB") INSURANCE / BOATING SAFETY & RELEASE OF LIABILITY ACKNOWLEDGMENT

I ________________ acknowledge that, if participating as a boater in all club events, I will maintain, and will present documentation verifying the minimum liability coverage ($300,000) for myself and my passengers as required by the club's insurance policy. I also acknowledge that not all insurance policies cover or insure tournament participation and, therefore, all participants in club activities must consult with their insurance company to confirm their individual coverage's while participating in club activities, including tournaments.

I also understand that (1) boating is a dangerous activity, and that safety must be observed during tournament practice and competition, and all other club events; (2) a U.S. Coast Guard approved a chest type life preserver (PFD.), Type III or higher, must be worn and properly fastened, and the engine's kill switch, with a lanyard no longer than 48 inches, must be attached to the driver whenever the combustible engine is operating; (3) all boats must be equipped with the recommended USCG safety devices; and (4) additional safety information concerning boating and fishing can be reviewed online at www.dbw.ca.gov

I further acknowledge that (1) as consideration for being a club member or guest participating in club activities, either as a boater or a non boater, including prior to, or during, an event, as well as post-event festivities, I agree to do so at my own risk; and (2) the club, its agents, and its sponsors shall not be liable for any damages, claims, or causes of action for personal injuries or property damages sustained in, on, or about the premises or waterways used for any club events or activities, including, but not limited to, dangerous or unsafe conditions at boat launches, bodies of water, and event parking lots or other public or private areas.

I further acknowledge and agree that I assume full responsibility for all injuries or damages of any kind or nature that may occur during any club event, regardless of cause, including those arising from the club's negligence or that of its agents, officers, board members, or sponsors.

My signature below acknowledges that I fully understand, and that I agree to comply with, the provisions set forth in this acknowledgment and understand (as outlined in the club by laws) that not carrying the minimum of $300,000 of liability insurance excludes me from participating as a boater in any club tournaments.
